<b>Alterações provocadas pelo papilomavírus humano nos exames citopatológicos em um laboratório escola da pontifícia Universidade Católica de Goiás</b>

Resumo

O Papilomavírus humano (HPV) é o agente etiológico do câncer do colo uterino. O objetivo deste estudo foi estimar a prevalência de alterações citológicas sugestivas de infecção pelo HPV, nos exames citopatológicos realizados no Laboratório Clínico da Pontifícia Universidade Católica de Goiás (LC-PUC-GO). Este foi um estudo quantitativo descritivo utilizando fichas de requisição dos exames citopatológicos encaminhados ao LC-PUC-GO. As variáveis foram categorizadas no programa Excel 2013 e utilizou-se o programa Epi Info™ versão 7 para avaliação entre as variáveis. Foram divididos em dois grupos etários (? 40 e >40) estratificado em menos graves: Atypical squamous cells of undetermined significance e low grade squamous intraepithelial lesion (ASC-US e LSIL), e mais graves: Atypical squamous cells of undetermined significance cannot exclude high grade squamous, intraepithelial lesion; High grade squamous intraepithelial lesion; Atypical glandular cells not otherwise specified; Atypical endocervical cells (ASC-H, HSIL, AGC-SOE, AGC-NEO e Adenocarcinoma), para o cálculo do Odds Ratio (OR), com intervalo de confiança (IC) 95% e significância p<0,05. Do total de 1.075 exames citológicos, a média de idade foi 42 anos. A faixa etária mais prevalente foi de 30 a 39 anos 25,12%. Nos exames citopatológicos foram observados 6,88% de anormalidades citológicas, sendo ASC-US 32% (24\74), ASC-H 25% (19\74), LSIL 18% (14\74), AGC-SOE/AGC-NEO 13% (10\74), HSIL 8% (6\74) e 1% (1\74) adenocarcinoma invasor. As anormalidades citológicas foram 1,86%, de 30 a 39 anos, seguida de 40 a 49 anos 1,58% e 14 a 19 anos 0,93%. Houve uma associação significante entre ? 40 anos e as lesões menos graves OR: 4,03 (IC: 95%: 1,34-12,40 p=0,04). As anormalidades podem ser corroboradas por mudanças de hábitos sexuais principalmente no início da atividade sexual precoce. O câncer do colo uterino pode ser previnível através da vacina anti-HPV e do exame citopatológico que auxilia na detecção e tratamento das lesões precocemente.

Changes caused by human papilomavirus in citopathologic ofa school laboratory of the Pontifical Catholic University of Goiás

Human papillomavirus (HPV) is the etiologic agent of cervical cancer. The objective of this study was to estimate the prevalence of cytological alterations suggestive of HPV infection in the cytopathological examinations carried out in a school laboratory in Goiás. This was a descriptive quantitative study using the records of requisition of cytopathological exams sent to the Clinical Laboratory of the Pontifical Catholic University of Goiás. Variables were categorized in the Excel 2013 program and the Epi Info ™ version 7 program was used for evaluation between the variables. They were divided into two age groups (? 40 and > 40) stratified into less severe: Atypical squamous cells of undetermined significance and low grade squamous intraepithelial lesion (ASC-US and LSIL), and more severe: Atypical squamous cells of undetermined significance can not exclude high grade squamous, intraepithelial lesion; High grade squamous intraepithelial lesion; Atypical glandular cells not otherwise specified; Atypical endocervical cells (ASC-H, HSIL, AGC-SOE, AGC-NEO and Adenocarcinoma), for the Odds Ratio (OR), with a 95% confidence interval (CI) and significance p <0.05. Of the total of 1,075 cytological exams, the mean age was 42 years. The most prevalent age group was 30 to 39 years 25.12%. In the cytopathological examinations, cyclosatological abnormalities were observed: ASC-US 32% (24\74), ASC-H 25% (19/74), LSIL 18% (14/74), AGC-SOE/AGC-NEO 13% (10\74), HSIL 8% (6\74) and 1% (1\74) invasive adenocarcinoma. The cytologic abnormalities were 1.86%, from 30 to 39 years, followed by 40 to 49 years 1.58% and 14 to 19 years, 0.93%. There was a significant association between ? 40 years and the less severe lesions OR: 4.03 (CI: 95%: 1.34-12.40 p = 0.04). Abnormalities can be corroborated by changes in sexual habits mainly at the onset of early sexual activity. Cancer of the cervix is preventable through cytopathological examination that assists in the detection and treatment of lesions early.
